Comments:
Thank you for your assistance during this unannounced inspection. This inspection was initiated by a complaint that was received by our licensing office on January 24, 2022, regarding the allegation in the area of staffing and supervision. The on-site visit was conducted on March 8, 2022 from 12:04 pm through 2 pm. The evidence gathered during this investigation supports the allegation, therefore the complaint is valid.

Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description: Based on interviews, the program did not ensure the care and protection of children.

Evidence:
1. The Program Director stated that they were made aware of a social media post on 1/13/22 claiming that a child in their care was given the wrong bottle.
2. Two former staff admitted the error to the Program Director when they were questioned.
3, Staff were retrained on proper infant feeding procedures.

Plan of Correction: The parents were called immediately and a lengthy discussion took place. Admin, the Director, and the parents agreed on a plan of action. Teachers were immediately retrained.
