Comments:
A monitoring inspection was conducted on 04/18/2023. There were 95 children present, ranging in ages from three months to five years, with 16 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of nine child records and nine staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description: SYSTEMIC DEFICIENCY

Based on the review of nine child records, the program failed to ensure that each enrolled child has a record including all required information for five children.

Evidence:
1. The record for Child #1 was missing the following: parent's work phone number, doctor's name and phone number, two emergency contacts, and signed written agreement.
2. The record for Child #2 was missing the following: proof of identity, first day immunizations, and physical.
3. The record for Child #7 was missing the following: sex, address, parent(s) name, address, and phone number, parent(s) work and phone number, doctor's name and phone number, two emergency contacts, authorization to pick up information, signed written agreement, proof of identity, immunizations, and physical.
4. The record for Child #8 was missing the following: sex, address, parent(s) name, address, and phone number, parent(s) work and phone number, doctor's name and phone number, two emergency contacts, authorization to pick up information, signed written agreement, proof of identity, and physical.
5. The record for Child #9 was missing the following: immunizations by the first day of attendance.
6. The administrator confirmed that the items were missing from the files and that they were currently working on cleaning up files and contacting parents regarding missing information.
