This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on Tuesday, March 30, 2021 and concluded on Wednesday, March 31, 2021. The in-charge person was contacted by telephone to initiate the inspection. There were 43 children present and 9 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Standard #: 22VAC40-185-60-A Description: Based on review of children's record, the center did not ensure to obtain all information for each child's record.
Evidence: In Child#1's record (start date 9/24/2020), the two designated persons to call in an emergency if parents cannot be reached addresses were incomplete.Plan of Correction: Center will ensure all information for each child's record is included, including emergency contacts addresses.
Standard #: 22VAC40-185-70-A Description: Based on review of staff records, the center did not ensure to obtain all information required for each staff record.
Evidence: In Staff#2's record (date of hire: 7/20/2020), the signature of person making the call for the reference check was not documented.Plan of Correction: Center will ensure all information for hiring is complete including signature of staff member conducting interviews/ reference checks.
