Comments:

An unannounced monitoring inspection was conducted on July 3, 2023 from approximately 1:45 pm to 3:30 pm. There were 38 children in attendance and a total of 9 staff present. 4 staff records and 4 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playgrounds. The children were observed reading books, and enjoying ice cream while the licensing inspector was on-site. Violations were cited as a result of this inspection. Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 07/18/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Description:	Based on a review of documentation, the center did not ensure to obtain a physical for each child within 30 days after the first day of attendance for 3 out of 4 children whose records were reviewed. Evidence: The record for child #1, date of enrollment 07/21/20, the record for child #2, date of enrollment 05/22/23, and the record for child #3, date of enrollment 02/02/22, did not contain evidence of an original physical exam or any updated physical exams.

Description:	Based on records review, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment or within the last 30 calendar days of the date of employment.
