An unannounced monitoring inspection was conducted on August 22, 2023, in response to a self-reported incident that was received on August 16, 2023 in reference to behavior guidance. Interviews were conducted and documentation was reviewed. The preponderance of evidence gathered during the investigation does not support violation of standards regarding the self-report.
