This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A subsidy health and safety (SHSI) inspection was initiated and concluded on 11/15/2021. There were 84 children present with 16 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-665-540-A Description: Based on review of five staff records, the center did not have documentation submitted of a tuberculosis test or screening at the time of employment and prior to coming into contact with children.
Evidence:
1. Staff #1 (Date of hire 6/09/2021) had a tuberculosis test or screening dated 6/14/2021.
2. Staff #2 (Date of hire 9/07/2021) had a tuberculosis test or screening dated 9/27/2021.
3. Staff #3 (Date of hire 6/13/2021) had a tuberculosis test or screening dated 7/02/2021.Plan of Correction: Will have TB results in hand before starting out of classroom training.
