Comments:
A subsidy health and safety (SHSI) inspection was completed. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Violations are identified on the violation notice.

Violations:
Standard #: 22.1-289.035-B-3
Description: The center must request a search of the central registry prior to the employee's first day of employment.
Staff #3, who had been employed for two months, did not have a central registry search requested. Staff #3 was working alone with children.

Plan of Correction: The central registry was mailed off on 8/29/24. A request will be made to OBI to check on, if it cannot be found a new one will be sent in.

Standard #: 8VAC20-790-540-B-11
Description: A written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.
Child #1, who has been diagnosed with a food allergy two months ago, did not have an allergy plan signed by the physician.

Plan of Correction: The parent has the form and will have the physician review and sign.

Standard #: 8VAC20-790-560-A
Description: Staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Staff #1, employed 14 months, staff #2, employed one month and staff #3, employed two months did not have a TB screen or statement at the time of employment.

Plan of Correction: Staff have been made aware that these screenings need to be completed.

Standard #: 8VAC20-790-600-B
Description: Within the first 90 days of employment all staff who work directly with children shall complete Virginia Preservice Training for Child Care Staff.
Staff #1, who has been employed for 14 months, did not have the Virginia Preservice Training on file.

Plan of Correction: All staff are currently working on this training. The director will ensure that all staff complete this training within 90 days of hire date.

Standard #: 8VAC20-790-600-D-1
Description: All staff who work directly with children shall have within 90 days of employment a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Staff #1, who has been employed 14 months, did not have a current CPR certification.

Plan of Correction: First aid and CPR was completed on July 3, 2024. No one have received their certificates yet. The Director contacted the trainer and the certificates will be sent.

Standard #: 8VAC20-790-600-D-2
Description: All staff who work directly with children shall have within 90days of the date of employment shall have a current certification in first aid appropriate to the ages of children in care.

Staff #1, who has been employed for 14 months, does not have a current first aid certificate.

Plan of Correction: First aid and CPR was completed on July 3, 2024. No one have received their certificates yet. The Director contacted the trainer and the certificates will be sent.

Standard #: 8VAC20-790-650-C-3
Description: The center shall have sinks located near the toilets and that are supplied with running warm water that does not exceed 120?F.
The water temperature in the downstairs sinks was 121.5 F and in the upstairs sinks was 133.3 F.

Plan of Correction: The director had the maintenance correct the water temperature.
