Discussion was held on the topics of current enrollment, current staffing, and training.
Discussion was also held on additional documentation to be required on the next birthday for household members #2 and #4.
Discussion that the facility has at least (1) one carbon monoxide detector installed on-site.

This monitoring inspection was initiated by licensing staff and concluded on 05/12/2022, between the hours of 10 am to 1 pm. There were 7 children present (25 points), ranging from infant to preschool-age, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 7 children records, 4 household records, and 2 staff records were reviewed.
Information gathered during the inspection determined that there was (1) one non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the home.

Standard #: 8VAC20-800-90-C Description: Based on review, 3 of 7 children files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for children (who were not exempt from the immunization requirements according to subsection B of this section), once every six months for children under the age of two years.
Evidence -
1. On the date of inspection (05/12/2022) the following children had immunization pages on file at the FDH old than 6 months: Child #3 (immunization page dated - 09/07/2021); Child #4 (immunization page dated - 03/30/2021), and Child #5 (immunization page dated - 10/26/2021).
2. On the date of inspection, based on information contain in their file, Child #3, Child #4, and Child #5 were all under 2 years of age.Plan of Correction: An updated immunization page will be obtained for all three children and placed in their file.
