An unannounced self-report investigation was initiated on 1/11/2024 due to a self-report complaint made to the Department of Health and Safety in the area of physical abuse. The investigation was completed on 01/24/2024. The inspector was on site on 1/11/2024 from 3:50pm until 5:00pm.
Two children's records and three staff records were reviewed. Interviews were conducted as well.
Information gathered during the inspection determined the complaint to be NOT VALID. Non-related violations were issued to the program. Violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-160-C Description: Based on record review, the center failed to ensure each staff member has a documented TB test or screening every two years.
Evidence: Staff #3 was working day of inspection on 1/11/2024. Staff #3 had a TB screening dated 09/14/2021.Plan of Correction: Not available online. Contact Inspector for more information.
