An unannounced monitoring inspection was initiated on 1/30/2024 and completed on 2/8/2024. The inspector was on site from 9:40am until 12:20pm.
There were 46 children under the supervision of 8 staff members. Children were observed during indoor free play, lunch and naptime. Four children's records and four staff records were reviewed. One staff member was interviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-270-A Description: Based on observation, the center failed to ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
1.In the preschool classroom, there was a plastic garbage over the children?s water fountain. Staff #3 stated the fountain has not been working. The plastic bag had been picked apart by the children.
2.In the school age classroom, there was a container on the floor underneath the classroom sink catching water that was dripping out of the pipe under the sink. The pipe was in need of repair. The water was stagnant.Plan of Correction: 1. The bag has been replaced and maintenance is being called to see if it can be fixed and if not, it will be removed.
2. Maintenance has been called to fix the sink. The Director has been told to keep the water emptied until this is fixed.
Standard #: 8VAC20-780-320-B Description: Based on observation and interview, the center failed to have supply bathroom sinks with running warm water that does not exceed 120?F.
Evidence:
In the preschool classroom, the hot water in the sinks would not turn on. There was no running water availablePlan of Correction: Maintenance has been called to look at this and if cannot fix will plumber.
Standard #: 8VAC20-780-350-B-2 Description: Based on observation, the center failed to maintain the required staff-to-children ratio for sixteen months to two year.
Evidence:
1.At approximately 9:40 am, 21 children were in care under the supervision of three staff in the two?s classroom. The ratio is one staff to five children. Four staff would be required.
2. At approximately 10:30 am, children from the two?s room were moved to another classroom. 18 children remained with 3 staff and the youngest was 21 months old. The ratio is one staff to five children. Four staff would be requiredPlan of Correction: This morning the heat in the infant/toddler classroom was not working so the director moved the children to other rooms and that caused the out of ratio. The Director came in at 10:30 to move the two's out.
Standard #: 8VAC20-780-370-1 Description: Based on observation and interview, the center failed to ensure that when an infant who falls asleep in a play space may remain in that space if comfortable and safe.
Evidence:
1.In the infant classroom, infants were observed sleeping on the bear mat on the floor. Children up to 19 months were observed walking and crawling around the children on the floor. Children were observed falling on and around the sleeping child on the floor. There were multiple children playing with toys in the area of the sleeping child.Plan of Correction: This morning the children were combined as due to the heat; the teachers have been explained how to handle this if they ever have a similar situation.
Standard #: 8VAC20-780-500-A Description: Based on observation, the center failed to ensure staff and children?s hands were washed with soap and running water after toileting.
Evidence:
1.Staff #5 was observed during the diapering process. There was no handwashing for child or staff after the diaper change.Plan of Correction: The PD will get training for staff #5 on diapering and handwashing procedures.
Standard #: 8VAC20-780-500-B Description: Based on observation, the center failed to ensure that there is a nonabsorbent surface for diapering or changing. Also, that disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot- operated or used in such a way that neither the staff member?s hand nor the soiled diaper touches an exterior surface of the system during disposal.
Evidence:
The diaper pale in the pre-school classroom was not foot operable. That part of the diaper pale was broken.Plan of Correction: Replaced the diaper pail with a new one.
