This inspection was initiated via telephone on 5/13/2020. The Inspector attempted to complete the remote inspection on 5/15/2020 but was unable to connect with the director/owner due to schedule conflicts. The remote inspection was completed on 5/18/2020.
- Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 5/13/2020 and concluded on 5/18/2020. The director/owner was contacted by telephone to initiate the inspection. There were 13 children present and 6 staff. The inspector emailed the director/owner a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and one violation was documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Evidence:
Child record B does not include the home phone number for each parent.Plan of Correction: I will ask the parent for the information.
