This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews .
A renewal inspection was initiated on 8/23/21 and concluded on 8/25/21. The director was contacted by telephone and a virtual inspection was conducted. There were 54 children present, ranging in ages from 6 months to 11 years old, with 16 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and four staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-191-40-D-1-A Description: Based on review of staff records and interviews the center failed to ensure that new persons designated as a board officer has a sworn statement within 30 days of appointment.
Evidence:
1. Board officer #2 does not have a sworn disclosure statement on file.Plan of Correction: The director will check with the main office to find out the status of these background checks. The center will ensure that all background checks on board officers are completed within the set time frames.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of staff records and interviews the center failed to ensure that staff have a central registry finding within 30 days of employment.
Evidence:
1. Staff #2 (hire date:6/14/21) and staff # 3 (hire date:12/7/20) do not have a central registry check on file.Plan of Correction: The acting director stated that #2 was mailed on 8/24 and #3 was mailed on 1/22/21 according to documentation found. The acting director will ensure that all staff have a current central registry check on file within the set time frames.
Standard #: 63.2(17)-1721.1-B-2 Description: Based on review of board officers files the center failed to ensure that board officers have submitted to fingerprinting background checks by the end of 30 days of service.
Evidence:
1. Board officer #2 does not have a fingerprint background check on file by the end of 30 days of service.Plan of Correction: The director will check with the main office to find out the status of these background checks. The center will ensure that all background checks on board officers are completed within the set time frames.
Standard #: 63.2(17)-1721.1-B-4 Description: Based on review of board officers records the center failed to ensure that Out of State Child Abuse and Neglect Search and Out of State Sex Offender Search were conducted on all board officers.

Board officer #2 does not have any of the out of state background checks required by the end of the 30th day of service.Plan of Correction: The director will check with the main office to find out the status of these background checks. The center will ensure that all background checks on board officers are completed within the set time frames.
