An unannounced monitoring inspection was initiated on 4/27/2022 and concluded on 5/16/22.
On 4/27/22 There were 48 children present with five staff.
The children were having a snack, playing with table games and outdoors.

Standard #: 8VAC20-780-340-A Description: Based on a review of staff files and staff interviews, the facility failed to ensure care and protection of each child in care.
Evidence:
1. Staff #1 reported that on February 16, 2022, during an attempt to quiet a group of children, staff #1 threw a water bottle, which was 1/4 full, toward the chalk board. The bottle hit child #1 on the side of the head. There was no injury.Plan of Correction: The Director of Operations reported that staff #1 was retrained in appropriate behavior guidance and received disciplinary action.
