10/30/23 Arrived 2:45pm Departed 4:20pm An unannounced monitoring inspection was conducted today. There were 82 children (ages 5-12yrs) present with seven staff . The children were arriving, departing, eating a snack, playing games. Five child records were reviewed. Two staff records were reviewed. Thank you for your cooperation. Standard #:	22.1-289.035-B-4
Description:	Based on the review of staff files, the facility failed to ensure that all staff complete all background checks within the required timeframe. Evidence: 1. There is no central registry background check on file for staff #2. hire date 9-5-23 REPEAT VIOLATION
Plan of Correction:	The area director reported that the background check was mailed on 10-5-23 and has not been received back from the central registry.
Standard #:	8VAC20-780-350-B-5
Description:	Based on observation and an interview with staff, the facility failed to ensure that the adult to child ration is maintained at all times. Evidence: 1. At 3:00pm , there were 48 children ages 5-12 years with two staff in the cafeteria. 2. At 3:15 a third staff member arrived.
Plan of Correction:	The site supervisor reported that a staff member had called out for the day, leaving the center without enough staff members. A staff member came from the Bristol office.
