An unannounced renewal inspection was conducted today.
There were 18 children (ages 20 mos- 10 years) present with seven classroom staff, a cook and an administrator.
The children were eating lunch, working in centers, resting and playing outdoors.
Four child records were reviewed. Staff records will be reviewed at the main office.

In addition to the renewal study, information was gathered for a self-reported incident.
A violation notice was issued.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on a tour of the playground, the facility failed to ensure that all areas and equipment are maintained in a safe condition.
Evidence:
1. On both climbers at the top of the slides, there have been pieces of composite decking boards (approximately 4inches by 8 inches) installed to close a head entrapment point. The gaps between the slide and the panels and between the pole and the panels present an entanglement point.
2. The bolts used to install these panels have more than 2 threads of the bolt showing presenting an entanglement point.

Plan of Correction: The program director reported that the playground equipment was repaired the day after the inspection to close the entanglement points.

Standard #: 8VAC20-780-340-A
Description: Based on information gathered through interviews, a video recording and a review of documents, the facility failed to provide care and protection of a child in care.
Evidence:
1. On July 12, 2023 at 4:43pm, child #1 was left on the playground, for approximately 2 minutes, after staff #1 and two other children went indoors.

Plan of Correction: Plan of correction:

1. People Incorporated placed the responsible staff person on administrative leave effective 6-13-23 and until the VDOE licensing investigation is complete.

2. On 6-13-23, Center Director verbally reminded staff of all sight and sound supervision procedures and reiterated that all staff are responsible for implementing these procedures and utilizing the provided walkie talkies when needed.

3. Program implemented additional procedure for playgrounds: ?Before exiting the playground, a second staff person will come out, check the playground, and assist with taking the children inside. At any time while on the playground if the staff member needs assistance, they will use the walkie talkie and call for help.? The Center Director provided instruction to staff on these new procedures and the new procedures were implemented on 6-13-23.

4. The Center Director or her designee will continue to monitor daily for compliance of all child supervision procedures. The Education Coordinator?s designee will continue to monitor weekly for compliance of child supervision procedures.

Date implemented: 6-13-23

1. Program added to child supervision procedures that staff who care for children in classrooms will utilize an attendance dry erase board, noting the number of children in attendance. This will be adjusted throughout the day as children arrive and leave.

2. Education Coordinator will provide a formal refresher training to center staff on all child supervision procedures including the addition of the playground exit plan and attendance dry erase board plan on 8-11-23.
