Comments:
An unannounced monitoring inspection was conducted on June 12 2023 between 9:30 and 10:30am. There were 70 children in the direct care of 10 staff members. Children were observed washing hands, playing outside, and going on walks. A review of children's records, staffing records, emergency drills, fire inspection, attendance records, and the physical plant was conducted. Standard #: 8VAC20-780-60-A
Description: Based on record review, the center did not ensure that each child enrolled have a separate record that contains the required information.

Evidence: Child 1,3, and 5 records did not have proof of identify verified by the center.

Plan of Correction: Will go to each parent and they will bring them in.

Standard #: 8VAC20-780-560-G
Description: Based on direct observation, the center did not ensure when food is brought from home that it is labeled and dated to identify the owner.

Evidence: In the toddler room, food did not have dates placed on them.

Plan of Correction: Will create their nametags and make sure they are on their lunch containers.
