Standard #: 8VAC20-780-270-A Description: REPEAT VIOLATION
Based on observation and interview, not all areas and equipment of the center were maintained in clean, safe, operable condition. Evidence: 1. In the bathroom of the 3's& 4's classroom, there was an unsecured, rolled up carpet in the corner. Staff interview revealed that the carpet was soiled recently and rolled up and stored in the bathroom used by children in care. There was a box of capri sun juice drinks in the bathroom cabinet under the sink.
2. In Preschool IA, the toilet paper holders were empty & not equipped with toilet paper. 3. In the SAC room, the toilet paper holders were empty & not equipped with toilet paper. There was a toilet brush in the top of a mop bucket in the middle of the restroom accessible to children. 4. On the two?s playground, the trashcan installed on the wall was overfilled with trash and the lid did not close.
4.In the infant room, in the crib of an eight month old, the purple sheet was oversized for the crib mattress and the material was bunched up. It was not tightfitting to meet safe infant sleep requirements.
5. In the infant room, there was a sleeping four month old infant improperly strapped in an infant swing. The straps were not over the infant?s shoulders. The straps were under the infants arms & were not properly secured as required. When the Licensing inspector inquired about this practice, no staff could provide an explanation & then one staff correctly strapped the child in. The straps were so tightly connected that the infant could not have removed their own arms & it would have been developmentally unlikely to have occurred.Plan of Correction: Not available online.
