An unannounced on-site renewal inspection was conducted on 2/14/2024. There were 34 children, ages 6 years-12 years, and 11 staff members present during the inspection. The inspector observed the following: arrival into the program, dinner, homework, computer lab and free choice activities. A total of 5 children?s records, five board officer/agents and 5 staff records, were reviewed. The inspector discussed the following with staff: background checks. The inspector arrived at 3:25 PM and departed at 5:40 PM. The inspection was amended on 2/26/2024.

Standard #: 8VAC20-780-70 Description: 3rd REPEAT VIOLATION
Based on review of staff records, the center failed to ensure that all records were complete per the standard.
Evidence:
1.Staff # 3 had a documented employment date of 2/12/2024 and the two required references were missing.
2.Staff # 4 had a documented employment date of 1/17/2024 and the two required references were missing.Plan of Correction: Three signatures will be required before any new staff/volunteer can start. Audit form used by Executive Director, Program Director and Office Manager followed by signatures from all 3 administrators.
Standard #: 8VAC20-780-280-B Description: REPEAT VIOLATION
Based on observation, the center failed to ensure that all chemicals were locked using a
safe locking method that prevents access by children.
Evidence:
1. A container of sanitizing wipes were sitting on a table in the cafeteria that was accessible to the children that were eating dinner.Plan of Correction: A walk through by the Program Director and Office Manager to ensure that the Center is in compliance with locked janitor storage area and all chemicals.
Standard #: 8VAC20-780-340-F Description: Based on interviews and observation, the center failed to ensure that all children under the age of 10 years.
Evidence:
1.Observed five children under the age of 10 years arriving off a city school bus and walking up the parking lot and ringing the doorbell to be let into the program. Staff was sitting at the desk and taking attendance when the children walk through the door. The children were not in sight and sound supervision from the bus to the inside of the building.
2.Observed three children under the age of 10 arriving in the parking lot next to the child care building and walking over through the parking lot to the child care building. The children had to wait until they staff answered the door. The two buildings are separated by large Evergreen trees.
3.Staff # 6 confirmed there were 23 children under the age of the 10 years that arrived off a city school bus and were out of sight and sound supervision for approximately 1-2 minutes.
4. According to staff policies; the staff are to be outside the doors in the parking lot to observe the children exiting the school bus.Plan of Correction: Administration will call the school to ensure that all children are dropped off at the correct parking lot to ensue that all children are within sight and sound with exiting.
Staff will be outside the door to watch the children exit the school bus and walk up the parking lot to the building.
Standard #: 8VAC20-780-560-K Description: Based on observation and interview, the center failed to ensure that the center scheduled all snacks or meals so that there is a period of at least 1-1/2 hours but no more than three hours between each meal or snack unless there is a scheduled rest or sleep period for children between the meals and snacks.
Evidence:
1.The last school bus arrived at 4:20 PM and as they arrive the children are served dinner. At 5:00 PM snack is served.Plan of Correction: Elementary age children will have snack at 6 PM.
