Comments:
An unannounced monitoring inspection was conducted with the assistant director on June 3, 2024, beginning at 9:46 a.m. and ending at 12:00 p.m. There were 24 children, ages between 9 months and 5 years old, in the direct care of 4 staff. The children were observed coloring and eating snack which consisted of apple slices, graham crackers, and water. Five children's records were reviewed and in compliance. Three staff records were reviewed and in compliance. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. The most recent practice emergency evacuation drill was documented as 12/23/2023. The most recent practice shelter-in-place drill was documented as 11/16/2023. The most recent practice lockdown drill was documented as 6/29/2023. The most recent health inspection was dated 1/10/2024. The most recent fire inspection was dated 1/3/2023.

Description: Based on a review of documentation, the center did not obtain documentation of a physical examination for each child enrolled in the program with in 30 days after the first day of attendance. Description: REPEAT VIOLATION: Based on observation and review of documentation, the center did not maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. On the fenced in playground, there was a broken, folding picnic table. In the four year old classroom, there was a broken door on the lower cabinet near the sink.

Evidence: In the school-aged classroom, a storage closet was left unlocked where 15 cans of paint, 3 cans of appliance epoxy, a bottle of carpet treatment/remover, a bottle of Stain Away, a bottle of carpet and rug dry cleaner, and a bottle of high performance enamel.
