This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.
An unannounced complaint investigation regarding preventing the spread of diseases was conducted on 11/10/2021. Interviews were conducted and policies and procedures were viewed.
