Technical Assistance:
Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
An unannounced, on-site renewal inspection was initiated on 11/12/2024 and completed on 11/12/2024. The on-site inspection began at 12:55PM and ended at 1:50PM. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 17. The inspector reviewed 3 children?s records and caregiver records. This inspection included document review, a tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/25/2024. A POC submitted after this date will not appear on the public website. Standard #: 8VAC20-800-780-B
Description: Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review.

Records for children #1 did not have a parent?s signature for an annual review of the emergency contact information. Child #1 had been in care for over a three years. The last annual review was conducted approximately 14 months ago.

Standard #: 8VAC20-800-100-A
Description: The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

There were no physical exam records available for Child #2 who had been in care for over 30 days.

Standard #: 8VAC20-800-80-A
Description: Within seven business days of the child's first day of attendance, the provider shall obtain from the parent verification of the identity and age of the child and name and location of previous day care programs.

This information was incomplete for child #2 and child #3, who have both been enrolled for approximately 1 month or less.

Standard #: 8VAC20-800-90-A
Description: Before attendance, documentation of immunizations is to be obtained for each child.

There was no documentation of immunizations for child #2, who was in care during the inspection and had been in care for a month.

Standard #: 8VAC20-800-520-A
Description: Cribs shall be provided for children from birth through 12 months of age and for children over 12 months of age who are not developmentally ready to sleep on a cot, rest mat, or bed.

3 children, over the age of 12 months, were sleeping in cribs. No documentation was available from parents to explain the children were not developmentally ready to sleep on a cot.
