An unannounced monitoring inspection was conducted today from 12;10-1:15. There were 8 children for a total of 21 points directly supervised by 2 staff. The home, 2 staff records, 5 children?s records, 1household member?s record, evacuation drills, emergency supplies, and policies were inspected. The entire house was walked through and the Provider stated only 2 people reside in the house. There were no medications on site today. Children were observed playing freely, eating lunch, and going down for nap. Diapering procedures were also observed. There was an assortment of age appropriate materials for children. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-800-60-A Description: Based on record review of 5 children's files, it was determined each file did not contain up-to-date information about services provided.
Evidence:
1. All 5 records had Medication administration policy stating two staff are authorized to administer over the counter meds and was signed by each parent.
2. The Provider and Caregiver #2 were listed as being MAT certified. The Providers MAT expired last year and Caregiver #2 is not Mat certified.Plan of Correction: I misunderstood the form as I saw current in First Aid and CPR and thought that was needed in the record. I also did not realize MAT had to be current for over the counter meds. I will document change and have families sign.
Standard #: 8VAC20-800-90-C Description: Based on record review of child #2's record, it was determined the Provider did not obtain documentation of additional immunizations for child #2.
Evidence:
1. Child #2 is under 2 years of age and the last documented immunization record is dated February, 2022.Plan of Correction: Will request current immunization record from the family.
Standard #: 8VAC20-800-510-A Description: Based on observation, a child was not provided with the correct resting equipment.
Evidence:
1. Child #2 was observed standing up in a crib. Child #2 is 2 years of age and has not been provided with a cot.Plan of Correction: Child was removed from the crib during the inspection and the child will be placed on a cot for sleeping.
