8VAC20-780-560 All food from home including water bottles if filled at home will need to be labeled with name and date.

An unannounced monitoring inspection was conducted on 5/3/23 with the director from 9:50 a.m. ? 12:15 p.m. Thirty-five children were present with six staff providing supervision. The ages of the children ranged from 18 months to 5 years. Children were observed singing, dancing, reading, coloring and engaging in circle time activities. Handwashing was also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Two medications, allergy care plans and medication documentation were reviewed. Three staff files and three children?s files were reviewed. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-80-A Description: Based on review of the written attendance record, the center did not ensure they maintained a written record of daily attendance for each group of children documenting their arrival and departure of each child in care as it occurs.
Evidence: In the Preschool classroom there were eleven children present and the written record reflected there were ten children present.Plan of Correction: The teacher assistant was shown the correct procedures. Also it was discussed with the entire team.
Standard #: 8VAC20-780-280-B Description: Based on observation the center did not ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.
Evidence: 1.The bathroom in the infant and toddler classroom was unlocked and the sanitizing spray was on the changing table accessible to children.
2. In the staff lounge there is no door and there were disinfectant wipes on the table accessible to children.Plan of Correction: 1. The proper procedure was reviewed with teacher and follow up.
2. Staff lounge door was secured with a safety.
Standard #: 8VAC20-780-430-C Description: Based on observation the center did not ensure that play equipment that met the requirement that openings above the ground which allow a 3.5 inch by 6.25 inch rectangle to fit through should also allow a 9 inch circle to fit through.
Evidence: On the play structure, under a platform above the ground there is a rectangular gap in the equipment that measures 5.5 inches by 32 inches. A 9-inch circle cannot fit through the opening which could lead to an entrapment of a body part or risk of strangulation.Plan of Correction: Will order new part for play equipment. Equipment will not be used until it is repaired.
Standard #: 8VAC20-780-510-E Description: Based on review of two medications and interview with staff the center did not ensure that they followed the policy for administration of medication.
Evidence: The medication for Child #1 did not contain an authorization from a physician or parent as requiredPlan of Correction: Immediately gave the proper paperwork to parent to get signature from medical official.
