8VAC20-780-420E3 Parents should at least annually provide confirmation that the required information in the child's record is up to date.

An unannounced monitoring inspection was conducted on 10/18/2023 from 10:00am to 11:35am. There were 40 children ages 16 months to age 5 years old supervised by 9 staff. The physical plant, outdoor playground area, programming, attendance, menus, 4 staff records, 3 children?s records, 2 medications with corresponding authorization records, emergency drills, injury reports, and policies were reviewed. Children were observed participating in reading, table toys, coloring, math and outside play. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-60-A Description: Based on review of 5 child records, the Center did not ensure they obtain all of the required documentation for child records.
Evidence:
1)The record for Child #3 did not contain authorization for emergency medical care should an emergency occur when the parent cannot be located immediately.
2)The record for Child #2 did not contain a written care plan for a diagnosed food allergy, to include
