An unannounced inspection was initiated on September 9, 2024 and completed on September 16, 2024 in response to a complaint received by the licensing office on September 5, 2024 relating to allegations of physical plant, programs, special care and provisions, and special services. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to staffing and supervision and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 23, 2024. A POC submitted after this date will not appear on the public website.

Based on observation and interviews; the center did not maintain required staff-to-children ratio.
Evidence:
1. On September 16, 2024, in room #1 at approximately 1:09 PM staff #5 went on break leaving staff #2 alone with 14 toddlers (16 months to 2 years old). Staff #3 entered the room after a few minutes.
2. Administrator #2 stated that staff #5 went on break at 1:09 PM.Plan of Correction: Not available online. Contact Inspector for more information.
