A complaint inspection was completed on 07/18/2024 in response to a complaint received by the licensing office 06/25/2024, relating to allegations of forbidden actions. This inspection included interviews and documentation review.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/02/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-410-1 Complaint related: Yes Description: Based on interviews and documentation review, it was determined that a staff used
the forbidden action of rough handling of children.
S1 and S2 were interviewed regarding incidents involving S3 being observed pulling a child, C1?s hair while walking the child from the playground to the building and a second incident with child, C2 by pulling the child by his arm in the classroom such that another staff intervened.
