An unannounced, on-site monitoring inspection was initiated on 09/10/2024 and completed on 09/10/2024. The on-site inspection began at 9am and ended at 10:30am. The inspector reviewed compliance in the areas listed above. There were 13 children present and two staff. The inspector reviewed five children?s records and two staff records on-site. This inspection included document review, tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/23/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-70 Description: Based on record review, the center did not ensure staff records contained all the required documentation.
Evidence:
1. Staff #1 did not have a completed orientation training on file.
2. Staff acknowledged the documentation was missing from the records.Plan of Correction: Office staff will make sure staff #1 complete orientation training documentation.
Standard #: 8VAC20-780-510-L Description: Based on observation, the center did not ensure all medication be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. Medication was observed located on top of a microwave in an unlocked backpack labeled with the child?s name.
2. Staff acknowledged the backpack was not locked and that a lockbox has been ordered.Plan of Correction: Center staff will ensure that medication will be kept in a locked place with safe locking methods to prevent access from children.
