Comments:

An unannounced monitoring inspection was conducted today. The provider and two assistant caregivers were caring for 9 children, totaling 21 points. Children's records, caregiver training, caregiver background checks, the physical plant, transportation, administrative records, emergency preparedness items, and caregiver:children interactions were inspected today. The children were observed walking to the neighborhood playground, playing with toys, engaging in creative play, and preparing for lunch. The children were to be served chicken nuggets, rice, apple slices, corn, and milk for lunch today. This inspection was completed between approximately 10:15am and 12:00pm. Standard #:	22VAC40-111-120-A
Description:	Based on a review of caregiver records, it was determined that the provider has not maintained a record for each caregiver. Evidence: The provider does not have a file for the assistant caregiver.
Plan of Correction:	She hasn't been officially hired. I will talk with her today to decide if she's going to be hired. If so, I will get her file together this week.
Standard #:	22VAC40-111-130
Description:	Based on caregiver interviews, it was determined that one assistant caregiver was not able to fully carry out all assigned job responsibilities. Evidence: Upon arrival, one assistant caregiver was present with 4 of the children in care. The provider was not home, and the substitute caregiver was out of the home with a group of 5 children walking to the neighborhood park. When the Inspector asked the assistant caregiver if she knew where the children's records were, she answered "no." When asked if she could contact a parent in the event of an emergency, she answered "no."
Plan of Correction:	I will make sure she gets trained to know where all administrative documents are.
Standard #:	22VAC40-111-210-A
Description:	Based on a review of caregiver training records, it was determined that the provider and one assistant caregiver do not hold satisfactory completion of First Aid and CPR. Evidence: Upon arrival, the assistant caregiver was alone with 4 children in care. This assistant does not hold current First Aid and CPR certification. The provider's First Aid and CPR certification expired on 10/31/2019.
Plan of Correction:	We will all take a First Aid/CPR course as soon as possible. In the meantime, the children will always be in the care of my substitute caregiver who does have current First Aid/CPR.
Standard #:	22VAC40-111-240-A
Description:	Based on observations, it was determined that one piece of equipment has not been maintained a safe and operable condition. Evidence: There is a small area rug in place at the entry door where children come/go and also play with toys. Two corners of this rug have curled up and now pose a tripping hazard for the children.
Plan of Correction:	I will get rid of that rug.
