An unannounced monitoring inspection was conducted Tuesday, February 4, 2020 to determine the center's compliance with licensing standards. The inspector arrived at 10:50 AM and concluded the inspection at 3:10 PM. Today, there were 91 children in attendance. During the inspection, children were playing outside, riding bikes, singing songs, taking bathroom breaks, washing hands, eating lunch, and napping. Children's equipment, learning materials, indoor and outdoor space, restrooms, first aid kit and nonmedical emergency supplies were reviewed. Medication is administered, and medication, authorization forms and allergy plans were reviewed. Ten children's records and ten staff records, fire drill, lockdown drill and shelter in place drill logs, daily attendance and all required postings were reviewed.
A member of management was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and can be found in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22VAC40-185-270-A Description: Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.
Evidence:
1. In classroom #1, the window sill had chipped paint and drywall showing.
2. In classroom #2, a blue cushion on the circle time bench had multiple tears and all four corners of the cushion was torn exposing foam.
3. In classroom #8, a foam funnel tube was ripped on all sides exposing foam.Plan of Correction: Room #1: A clean safe environment is one of our top priorities. The maintenance department is made aware of the chipped paint and drywall showing and corrections will be made.
Room #2: Before the inspection, cushions were ordered, but when they arrived they were the wrong size. A second order was placed, and we were awaiting the arrival when the inspection occurred. The cushions were removed immediately from the classroom and have since been replaced. The Director and staff will continue to monitor the classrooms for health and safety compliance.
Room #3: The funnel tube was thrown away immediately. The Director and staff will continue to monitor the classrooms for health and safety compliance.
Standard #: 22VAC40-185-280-B Description: Based on observation, the center did not ensure that all hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
In classroom #5, the unlocked closet had disinfectant wipes labeled "Caution"; "Keep out of reach of children"; and "Hazards to humans," and GermX labeled "Flammable" and "Keep out of reach of children." The closest was accessible to children.Plan of Correction: Although the cleaning supplies were in an unlocked cabinet, the closet door was closed and the chemicals were out of reach of children. The staff member immediately locked the closet door, and the Director and staff will continue to monitor the classrooms for health and safety compliance.
Standard #: 22VAC40-191-40-D-6 Description: Based on staff record review, the center did not ensure that satisfactory fingerprint-based national criminal background checks are not dated more than 90 days prior to the date of employment.
Evidence:
The record for staff #1 (date of hire 01/30/2020) had documentation of fingerprint-based national criminal background check dated 10/25/2019.Plan of Correction: On date on inspection 2/4/2020, the staff member out of compliance resubmitted another background check. Skipwith Academy at Woodlake received the eligibility letter for the staff member on 2/7/2020. The Director will ensure that all new hire forms are completed and received within 90 days prior to hire or must be resubmitted before staff member may begin employment.
