An unannounced monitoring inspection along with an investigation of a self reported incident and later complaint regarding daily activities, supervision and care and protection and staff qualifications was conducted on 11/18/2019 and 12/10/2019. On the later date there were 23 children (infants-5yrs.) directly supervised by 8 staff. Interviews were conducted, The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. During the inspections children were observed participating in group play, reading books, and eating snack. Diapering and hand-washing procedures were also observed. There was an adequate supply of materials for the children. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-185-60-A Description: Based on review of child records it was determined that all required information was not maintained.
Evidence: During inspection Child 1 was missing parents' work phone number Child 2 was missing phone numbers for 1 of 2 emergency contacts.Child 3 was missing work numbers for 1 of 2 parents listed.Plan of Correction: Parents were contacted during inspection.
Standard #: 22VAC40-185-70-A Description: Based on review of staff files it was determined that CPR and First Aid certifications for teachers were not maintained.
Evidence: During inspection Teacher A did not have a current CPR and First certification available.Plan of Correction: Teacher stated that updated documentation was misplaced and duplicate will be requested.
Standard #: 22VAC40-185-280-B Description: Based on inspection of Facility it was determined that hazardous substances were not kept in a locked place as required.
Evidence: 1. During inspection the cabinet below the sink in the Penguins classroom was unlocked containing cleaning agents .
2. During inspection the closet in the cubs room was unlocked containing cleaning supplies.Plan of Correction: During inspection a lock was installed and applied
Standard #: 22VAC40-185-380-A-2 Description: Based on interviews with staff it was determined that the required span of time allotted for nap time was exceeded.
Evidence: On both dates 6 of 8 staff stated that rest time is from 12:00pm - 3:00pm totaling 3 hours rather than the 2 hours required.Plan of Correction: Not available online. Contact Inspector for more information.
