The Licensing Inspector initiated and completed an unannounced monitoring inspection on 3/16/2023. The inspector was on site from 10:20am until 12:35pm.
There were 37 children under the care of 6 staff. The ages of the children ranged from four months to five years old. The children were observed during indoor free play, outdoor play, and lunch. Four children?s record, one medical record, and three staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-140-A Description: Based on record review, the center failed to obtain a copy of the child physical examination before child?s first day or within 30 days after first day of attendance.
Evidence: Child #2 has an enrolled on 1/23/2023, had no documentation of a physical on file during the inspection on 03/15/2023.Plan of Correction: A copy of the physical will be obtained by 4-7-2023. PD will continue to review files for completion upon enrollment and thereafter for compliance.
Standard #: 8VAC20-780-270-A Description: Based on observation, the center failed to maintain areas and equipment of the center in a clean, safe operable condition.
Evidence:
A metal bracket used to hold the wooden boarder together on the playground had one end of the bracket sticking out approximately an inch from the boarder. The metal was sharp and rusted. This causes a hazard to children. Children were observed playing in this area.
The wooden boarder on the playground has split apart at the corner causing a three-inch gap. This causes an entrapment hazard for children. Children were observed playing in this area.
On the wooden porch leading out of the classroom to the playground, the railing on the porch has separated in the corner causing approximately a one-inch gap exposing the nail in the rail. This creates a snagging hazard for children. Children were observed playing in this area.Plan of Correction: The metal bracket has been fixed 3/17
The wooden boarder will be relaced by 4-10.
The railing has been fixed as of 3-17
Staff will be reminded to inform the PD immediately of any safety issues for continued compliance and safety.
