Comments:
A renewal inspection was conducted on 06/29/2022. There were 90 children present, ranging in ages from 5 to 12 years, with 15 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Violations:
Standard #: 8VAC20-780-510-B
Description: Based upon review of children's medication, the facility has not ensured that written parental permission is obtained if medication is to be administered.
Evidence:
1. The parent of child 1 had not signed the allergy action plan giving the facility permission to administered the child's albuterol if needed.
2. Staff 1 acknowledged that the parent had not signed the allergy action plan giving the facility permission to administer the medication.

Plan of Correction: The facility responded with the following:
A form has been printed on which the parent will be required to sign authorizing the medication to be administered by Horizons' staff. The form includes the duration/length of time for which the parent is giving authorization

Standard #: 8VAC20-780-510-E
Description: Based upon review of children's medication, the facility has not ensured that their procedures for administering medication include the duration of the parent's authorization for the administration of the medication.
Evidence:
1. The medication record for child 1 did not include the duration of the parent's authorization for the medication to be administered.
2. Staff 1 acknowledged that the allergy management plan form for child 1 did not include the length of time the authorization for the medication to be administered was being given.
