Comments:
An unannounced monitoring inspection was conducted on May 24, 2024 approximately 12:30 pm to 1:30 pm. There were 9 children for a total of 7 points directly supervised by 1 staff. The home, 1 staff records, 2 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely. There was an assortment of age appropriate materials for children.

Description: Based on children records review, 2 out 2 records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #1 and Chidl #2 did not have a physical examination in their record prior to a child's attendance or within 30 days after the first day of attendance.

Plan of Correction: The parent has an appointment with the doctor on 6/14/2024

Standard #: 8VAC20-800-180-A
Description: Based on household and staff records review, 2 out 2 records, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
1. Staff 1 Tuberculosis Screening form exoired on 3/15/2024.
2. Household memeber 1 Tuberculosis Screening form expried on 3/22/2024,
