Comments:
An unannounced monitoring inspection was conducted on August 11, 2023 from approximately 11:30 am to 1:30 pm. There were 57 children in attendance and a total of 11 staff present. 6 staff records and 6 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. The children were observed getting ready for lunch, watching a movie, and taking a nap while the licensing inspector was on-site.

Violations were cited as a result of this inspection.

Please sign and date and complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 08/29/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Violations:
Standard #: 8VAC20-780-40-E
Description: Based on observation, interview, and record review, the center failed to maintain compliance with the terms of the current license issued by the department. Evidence: The terms of the license state that children under the age of 31 months must remain on the first floor in rooms with direct egress. Child #4 was observed in classroom #2, upstairs. Classroom #2 does not have direct egress. When interviewed, staff stated that child #4 was the youngest child in the class. Record review revealed that child #4 is 30 months old.

Plan of Correction: Per the center "children will not move upstairs or visit until 31 months."

Standard #: 8VAC20-780-370-1
Description: Based on observation, and interview, the center failed to follow a flexible daily schedule that allows sleep as needed for children ages birth through 16 months. Evidence: At approximately 12:30 pm all of the infants were placed in their cribs, the lights were turned out, and soft music was playing. Staff were actively trying to get all of the infants to fall asleep at the same time by rubbing their backs and laying them back down when they tried to stand up. When asked staff stated that this is a daily practice and that all of the infants are expected to take a nap at the same time every day.

Plan of Correction: Per the center "we will put the children down as they're showing signs of needing a nap."
