Comments:
An unannounced investigation was conducted in response to a complaint that was submitted to the Fairfax Licensing Office, regarding physical plant, ratios, and supervision. Observations, interviews, and information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented related to supervision. There was not a preponderance of evidence to support the allegation regarding ratios or physical plant concerns.

Complaint related: No
Description: Based on interview and review of documentation it was determined that staff did not always ensure that children remained within sight and sound supervision.

Evidence:
1. On 10/16/2023, at approximately 9:20am, Child #1 (15 months old) was placed in a crib near the front desk with Staff #1, to wait for their parents to pick them up.
2. On the day of the incident, Adult #1 reported that as they were leaving the facility, they observed Child #1 in a crib near the front desk alone and unsupervised.
3. Staff #2 and Staff #3 stated that on the day of the incident they observed Child #1 in a crib by the front desk, alone and unsupervised. Staff #2 reported that Staff #1 stepped away from the desk and walked into a classroom, leaving Child #1 unsupervised for approximately 2 minutes.

Plan of Correction: The evening of 15 November during a Professional Growth and Development training, all caregivers - teachers (which includes Staff #1) were reminded of the importance and critical nature of sight and sound supervision as part of insuring the health, safety, welfare and well-being of all children enrolled at the FBCC CDC when in attendance.
Strategies for making certain that sight and sound supervision is apparent to all in the building including when a caregiver - teacher's presence may not be obvious to authorized adults, other caregivers - teachers in the building were reviewed and will be implemented moving forward.
As reported by Staff #2, Staff #1 stepped away from the desk and walked into a care setting. Although outside of Adult #1's sight because of the location of the care setting into which Staff #1 had stepped, Staff #1 is certain they were and remained in sight and sound supervision of Child #1 for the period of time Staff #1 had stepped away from the desk and into the care setting was able to see and hear Child #1 as the door to that care setting was partially opened with a clear line of vision to Child #1.
