Comments:

An unannounced monitoring inspection was conducted on January 3, 2023 from approximately 10:00 am to 12:30 pm. There were 61 children with 11 staff directly supervising. 6 staff records and 6 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. Children were observed playing with playdough, practicing writing letters, reading, and playing outside while the licensing inspector was on the premises. The most recent health inspection was done on 12/6/22. The most recent fire inspection was completed on 12/28/22. Emergency drills were conducted monthly with the most recent completed on 12/13/22. The first aid kit was inspected and found complete. The center had a working flashlight and radio. Violations were cited as a result of this inspection. Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today, 01/10/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Description:	Based on records review, and interview, the center failed to maintain the required documentation in the staff record for each staff person. Evidence: 1) The records for staff #3, date of employment 8/22/22, and staff #6, date of employment 10/25/21, did not contain documentation that two or more references had been checked. 2) 5 out of 6 records reviewed did not contain documentation of education, certification or experience required by the job.

Description:	Based on records review, and interview, the center failed to ensure that the Virginia Department of Education (VDOE) sponsored orientation course was completed for all staff within 90 calendar days of employment. Evidence: 1) The record for staff #1, date of employment 09/26/22, and the record for staff #5, date of employment 1/17/22, did not contain documentation of the VDOE course being completed. When interviewed, Administrators confirmed that the trainings had not been completed. 2) The record for staff #6, date of employment 10/25/21, contained documentation of the VDOE training dated 3/15/22. The training must be completed within 90 calendar days of employment.

Description:	Based on observation, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: The bathroom nearest classroom #3 has a sink that is separating from the wall and there is a hole in the wall approximately 1 inch by 3 inches below the sink.

Description:	Based on interview, the center failed to ensure that the diaper surface is cleaned with soap and at least room temperature water and sanitized after each use. Evidence: When interviewed, staff #5 stated that they do not clean and sanitize the diaper surface after each diaper change. They spray the surface with Lysol spray. Administrator?s confirmed that this is the diaper changing process.
