Remove Lamp in school-aged restroom plugged into the wall next to the sink and clean up water observed on the school-aged restroom floor following water play day.
- Comments:

An unannounced, on-site monitoring inspection was initiated on 7/24/2024 and completed on 7/24/2024. The on-site inspection began at 9:57 a.m. and ended at 1:25 p.m. The inspector reviewed compliance in the areas listed above. There were 35 children present and six staff. The inspector reviewed six children?s records and six staff records on-site on 7/24/2024. This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/8/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-270-A Description: Based on observations, areas and equipment of the center, inside and outside, were not maintained in a safe and operable condition.
Evidence:
1. Broken floor tiles and split flooring was observed throughout the school-age classroom. Tape was used previously to conceal the damaged flooring. The tape was broken, exposing tripping hazards.
2. Damaged vents were observed in the School-age Classroom and the Two-Year-Old Classroom. The vents were bent and lifting from the floor surface.
3. In the Pre-School Classroom there was chipping paint along the windowsills and along the wall.
4.On the outside playground, metal fencing surrounding the perimeters had several areas of untwisting wiring, resulting in an unraveling-spoke; a broken panel on the wooden fence; nails were loose and lifting on the wooden fence.
5. On the outside porch, there was a broken table, accessible to the children outside the Preschool and School-age Classrooms.Plan of Correction: All repairs will be completed no later than August 11, 2024. The facility is currently in the process of adding classrooms to the building and renovating the current building. The process is taking longer than first thought. The director will initiate a new process to be notified of repairs needed. The staff and children were aware the picnic table was not available for use. It will be removed immediately
Standard #: 8VAC20-780-280-B Description: Based on observations, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the Toddler room, in a cabinet, located in the corner of the room, there were two sanitizing solutions, and an aerosol can room spray.
2. In the Two-Year-Old restroom, in a cabinet above the countertop, there was a bottle of sanitizing solution. The cabinet?s lock was broken.
3. On the outside playground porch, there was an open bag of garden soil. Manufacturer?s warning label stated, ?keep out of reach of children?.Plan of Correction: The lock on the cabinet in the toddler room was available but it had been forgotten to be put back on after cleaning tables that morning. The staff will be reminded to replace the child safety lock after every use. The lock on the two-year old restroom cabinet over top of the changing table will be repaired no later than August 11, 2024. The bag of soil was put in a storage box out of reach of the children on the same day as the inspectors visit.
Standard #: 8VAC20-780-500-B Description: Based on observation, the center did not provide a storage
system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
Evidence:
1. The trash can lid in the Infant Room was broken. Staff verified that they had to use their hands to remove the lid with their hands to dispose of the soiled diapers.
2. The trash can lid in the Two-Year-Old Classroom was broken, and on the floor, there were soiled diapers/pull-ups in the trash can.Plan of Correction: The trash cans were replaced immediately, as extra trash cans were in the building. The director will initiate a new process to be notified of repairs needed.
