An unannounced inspection was initiated on 9/17/24 and completed on 10/9/24 with a local agency in response to multiple complaints received by the licensing office between 8/21/24 and 9/12/24 relating to allegations of supervision, cleanliness and building maintenance. This inspection included document review, tour of the facility, interviews and observations. The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non- compliance with applicable standards or law and the violations are documented on the violation notice issued to th program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/31/24. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-270-A Complaint related: Yes Description: The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. The carpet in the preschool area was separated at the seam and was frayed. There were stains on a crib mattress. Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-510-L Complaint related: No Description: Medication, except for those prescriptions designated otherwise by written physician's order, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children. In the infant room there was a bottle of Infant Motrin in a diaper bag that was at a height that was accessible to children. Plan of Correction: Not available online.
