An unannounced, on-site monitoring inspection was initiated on 7/17/2024 and completed on 7/17/2024. The on-site inspection began at 11:05 am and ended at 12:25 pm. The inspector reviewed compliance in the areas listed above. There were 28 children present and 6 staff. The inspector reviewed 5 children?s records and 7 staff records on-site. This inspection included a document review, a tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/23/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-140-A Description: Based on record review and staff interview, 1 of 5 children's records indicated the center failed to ensure that all physical examination records were obtained before the child's attendance or within 30 days after attendance.
Evidence: There was no documentation of a physical examination for Child #2 whose first date of attendance is 11/28/2023.Plan of Correction: Parent/s of child will be conducted to bring Physical Examination report from physician by the end of the day of inspection.
Standard #: 8VAC20-780-260-B Description: Based on record review and staff interview, the center failed to obtain annual approval from the health department for meeting requirements for water supply and sewage disposal system.
Evidence: There was no documentation of an annual environmental sanitation inspection being conducted.Plan of Correction: Loudoun County Health Department will be contacted before the end of the day of inspection to get written confirmation of scheduled date of annual environmental inspection.
