An unannounced renewal inspection was conducted on June 9, 2023 from approximately 11:15 am to 1:00 pm. There were 38 children in attendance and a total of 5 staff present. 4 staff records and 4 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. The children were observed eating lunch and getting ready for nap while the licensing inspector was on-site.
Evacuation drills were conducted monthly. The most recent health inspection was conducted on 01/13/2023. The most recent fire inspection was conducted on 11/24/2022.
Violations were cited as a result of this inspection.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 06/14/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Standard #: 22.1-289.035-B-4 Description: Based on records review, the center failed to obtain an out of state criminal history record information check, and a sex offender registry check prior to employment, and a search of the child abuse and neglect registry by the end of the 30th day of employment for any state an employee has lived in within the 5 years prior to employment. Evidence: The record for staff #2, date of employment 06/15/2022, indicated that staff #2 lived in another state within the 5 years prior to employment. The record did not contain the results of a criminal history record information check, a sex offender registry check, or a child abuse and neglect search from that state. Plan of Correction: Per the center "going forward I will attempt to file and receive out of state background on new hires."
Standard #: 8VAC20-770-60-C-2 Description: Based on records review, the center failed to obtain the results of the Central Registry finding by the end of the 30th day of employment. Evidence: The record for staff #1, date of employment 01/17/2023, did not contain the results of a Central Registry search. Plan of Correction: Per the center "as of now I will make sure I have CRS on new hires."
Standard #: 8VAC20-780-70 Description: Based on records review, the center failed to maintain all required documentation in staff records. Evidence:
The record for staff #1, date of employment 01/17/2023, did not contain documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position.Plan of Correction: Per the center "as of today I will make sure to have a TB test on each new hire."
Standard #: 8VAC20-780-270-A Description: Based on observation, the center failed to ensure that areas and equipment inside and outside shall be maintained in a clean, safe, and operable condition. Evidence:
1) Classroom # 1 had a broken toilet which required the back cover to be taken off in order to be flushed.
2) Classroom #2, contained a broken cabinet that was hanging off the hinge, creating a pinch hazard. The cabinet was low and able to be accessed by the children, age 16 months to 24 months.Plan of Correction: Per the center "1 has been fixed. 2 a service ticket has been sent to corporate to have it fixed."
