During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced monitoring inspection was initiated on 10/23/2024 and completed on 11/08/2024 in response to self-reported incidents received by the licensing office on 10/15/2024 and 10/18/2024 relating to special feeding needs, and supervision of children. The inspector reviewed 2 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self-report, violations that were not reported were found related to staff training orientation, child records and injury reporting are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/15/2024. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-140-A
Description: Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of Child #1, who had been enrolled for three months.

Plan of Correction: -Moving forward the center's administration will ensure that all children in the program have the required physical examination in their files, within the first 30 days of the child's enrollment.

Standard #: 8VAC20-780-240-B
Description: REPEAT VIOLATION
Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.

Staff #3, who has been working with children for 16 months, did not complete orientation training.

Plan of Correction: Making sure new hires complete the orientation and sign the orientation checklist within the first 7 days of hire.

Standard #: 8VAC20-780-340-A
Description: When staff are supervising children, they shall always ensure their care, protection, and guidance.

1. On 10/15/2024, Staff #1 fed Child #1, 1.5 oz of breast milk, from a bottle belonging to Child #2. Child #1 has a medical condition and special feeding instructions on record.

2. On 10/16/2024, Staff #2 dropped Child #1, age 15 months, from shoulder height while standing. Child #1 landed on the side of their head and shoulder area. Child #1 required outside medical attention and had bruising between the neck and shoulder area.

Plan of Correction: - Adding an additional step to handling and storing the babies' bottles which is checking the bottles with the parents during the drop off.
- Provided training on Safe Handling and Storage of Baby Bottle and Training cups.
- Provided training on Infants' Feeding policy.
-Both individuals that were involved in both incidences were terminated.

Standard #: 8VAC20-780-550-N
Description: The center shall notify the parent immediately if a child requires emergency medical treatment or sustains a serious injury.

On 10/16/2024, Child #1, age 15 months, was dropped and fell headfirst from shoulder height of Staff #1 to the floor. The parents were not notified immediately. Staff assessed Child #1 in the classroom and then waited for another staff to return from break and wrote a report before informing the parents, reportedly 50 minutes after the injury. The child was hospitalized when taken for additional examinations by the parents.

Plan of Correction: -Provided training on reporting accident/incident, and steps to completing an accident/incident report.
- Provided training on Safe Infant and Toddler Lifting Policy.
- followed with one-on-one meetings with all the teachers, provided them with two copies of the policies, one of which was signed and kept in their files, and the other to keep as a reference.
-The individual who was responsible for the incident was terminated.

Standard #: 8VAC20-780-570-E
Description: REPEAT VIOLATION
Prepared infant formula shall be dated.

During the inspection on 10/23/2024, 3 out of 7 bottles containing prepared infant formula were not labeled with the date.

Plan of Correction: - Conducting a routinely check for
the infants' bottles.
- Provided training on infants feeding practices.
- Had teachers review and sign the Infants' Feeding Policies.
