Comments:
An unannounced Monitoring Inspection took place on August 30, 2022 between the hours of 12:00 PM and 2:35 PM. There were 5 classrooms observed with a total of 31 children with 7 staff supervising within the supervision guidelines. The children were observed finishing lunch, preparing for naptime, going to the restroom, handwashing, looking at books and resting on their cots. A complete inspection of the physical plant, children and staff records, fire drill log, programming, first aid kit, medication and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, areas outside are not maintained in a clean and safe condition.
Evidence:
1. Splinter logs laying around on the preschool playground near play equipment.
2. Dirty stagnated water with toys inside a bucket on the playground.
3. Several large piles of wood and dried weeds laying around on both playground areas.
4. Wood around the sandbox is splintered and in need of repair or replacement.
5. Protective chain coating on the swings are broken and splintered.
6. Standing water in rusty pans on playground wooden table.

Plan of Correction: I will tell the administrators of the property this has to be corrected.

Standard #: 8VAC20-780-290-A-3
Description: Based on observation, three (3) electrical outlets did not have protective covers as required in areas used by children of preschool age or younger.
Evidence:
1.Three classrooms (Caterpillars, Dragon Fly, and Kinders) were missing an outlet cover.

Plan of Correction: I will have them covered now. Correction was made during inspection.

Standard #: 8VAC20-780-330-B
Description: Based on observation, the playground did not have resilient surfacing to meet the minimum safety standards for play areas and did not have a fall zone free of hazardous obstacles around equipment.
Evidence:
1. Mulch barely covering the dirt under and around the swing/slide and climbing equipment.
2. Hazardous obstacles (large tree stumps and logs) were within 3-4? of swing equipment.

Plan of Correction: The mulch is in the parking lot. I will tell them again, we need it placed on the playground.
