Licensing standards regarding documentation of children's injuries and best practice was discussed with the program director on 01/24/2020. A model injury record was given to the program director. This model form may be adapted however the facility may choose. The facility is not required to use this form.
- Comments:

A complaint investigation of alleged inadequate supervision resulting in an injury of a child was investigated on 12/10/2019, with follow-up on 1/24/2020. The investigation included staff interviews and review of documentation. After a thorough review of the evidence, the complaint is determined to be not valid as the facts do not show by a preponderance of evidence that insufficient supervision occurred.
Results of the investigation were shared with the program director on 02/28/2020.
