Comments:

An unannounced monitoring inspection was conducted today from 9:35am-11:00am. There were 6 children directly supervised by 2 staff. The physical plant, outdoor playground area, programming, menus, 2 staff records, 3 children?s records, emergency drills, emergency supplies, and policies were inspected. No medications were on site at the time of inspection. Children were observed completing their morning journals, engaging with staff and peers during circle time, singing songs and learning about months, letters, and numbers. Child also participated in a matching activity while reading through the Hungry Caterpillar book. Children transitioned to snack time by washing hands first and then eating morning snack which consisted of grapes, crackers, and water. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on 6/2/2022. Description:	Based on observation and staff interview, the center did not ensure that the program was equipped with at least one carbon monoxide detector. Evidence: There was not a carbon monoxide detector installed at the center at the time of the inspection.
Plan of Correction:	They have already been installed in all classrooms.
Standard #:	8VAC20-770-60-C-2
Description:	Based on record review, the center did not obtain the results of a Virginia Child Abuse and Neglect Central Registry search within 30 days of employment for all staff. Evidence: Staff #2 (Date of Hire: 8/16/2021) did not have Virginia Child Abuse and Neglect Central Registry search results on file at the time of inspection.

Description:	Based on record review, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: Child #2 was present at the center during the inspection and did not have immunizations on file.

Description:	Based on record review, the center did not obtain documentation of a physical examination for all children prior to attendance or within 30 days after the first day of attendance. Evidence: Child #2 (Start Date: 1/25/2022) was present during the inspection and did not have a physical examination on file.

Description:	Based on record review, the center did not ensure that all children's records contained all required information. Evidence: Child #2's record was missing names, addresses, and phone numbers for two designated people to call in an emergency if a parent cannot be reached and documentation of viewing proof of the child's identity and age.
