An unannounced monitoring inspection was conducted on 8/17/23 from 12:56 p.m. ? 3:38 p.m. with the administrator. There were 50 children in care, ranging in age from four months to 12-years-old, supervised by six staff. The children were observed napping, eating snack and free play. The physical plant, outdoor playground area, programming, attendance, menus, first aid kit, and emergency drill log were reviewed. Diapering and handwashing procedures were also observed. Four child records and three staff records were reviewed. Required postings were observed. Areas of non-compliance are identified in the Violation Notice. Standard #: 8VAC20-780-40-M Description: Repeat Violation
Based on observation and interview, it was determined that the center did not maintain a current written list of all children?s sensitivities and dietary restrictions.
Evidence:
1. In the toddler class, Child #2 had three bottles of oat milk in the refrigerator, staff stated the child had a milk sensitivity. The sensitivity was not listed on the allergy/dietary/food sensitivity list dated 2/9/2023 posted in the classroom.
2. In the School Age class, teacher stated that two children had milk sensitivities, the sensitivity was not listed on the allergy/dietary/food sensitivity list dated 2/9/2023.
3. In the Early Preschool class, staff reported that three children in the class had milk sensitivities, on the allergy/dietary/food sensitivity list dated 2/9/23 the children were not listed. The teacher wrote the children?s names on a post it note and posted it on the outside of the cabinet.Plan of Correction: We did have current written list of children sensitivity posted. However teachers will not accept non-documented milk
Standard #: 8VAC20-780-270-A Description: Based on observation, not all areas of the center were maintained in a clean, safe, and operable condition.
Evidence:
1. In the Early Preschool, Preschool, Pre-K, and School Age bathrooms there were screws that extended from approximately seven thread lengths to 22 thread lengths at the base of the toilets, in each class, were exposed and create an impalement and scratch hazard.
2. On the Infant and Toddler playground the resilient surfacing under the caterpillar had a large chunk of the surfacing missing causing a trip hazard.
3. In the School Age class a speaker was placed on the half wall that is shared with the Preschool classroom, the cord to the speaker was hanging down on the school age side. The cord dipped down across the library area, within reach of the children, creating a fall hazard.Plan of Correction: knobs came off but now replaced
Standard #: 8VAC20-780-330-B Description: Repeat Violation
Based on observation, it was determined that there was not at least six inches of resilient surfacing in the fall zones of the playground equipment.
Evidence: On the older playground, the licensing inspector observed a thin layer of mulch, approximately one inch, underneath and surrounding the merry-go-round, jungle gym, and five seat trike set on the playground.Plan of Correction: We will continue to resurface the mulch. However we are looking in pour in place.
Standard #: 8VAC20-780-340-A Description: Based on observation, it was determined that staff did not ensure care and protection of all children.
Evidence: In the early preschool class, a two year old child was observed sleeping on a cot with a sippy cup in their mouth. This poses a potential choking hazard.Plan of Correction: Child was asleep with bottle. However all children will sit up on chair to drink.
Standard #: 8VAC20-780-500-A Description: Based on observation, hand washing procedures after toileting were not always followed.
Evidence: At the time of inspection, Licensing Inspector (LI) observed a teacher returning a child to the group after changing a soiled diaper without washing the child?s hand or their own hands with soap and running water.Plan of Correction: Teacher is trained in hand washing and sanitization
Standard #: 8VAC20-780-500-B Description: Based on observation, the diapering surface was not always being cleaned with soap and water and sanitized after each use.
Evidence: In the toddler class, the LI observed a teacher completing a diaper change and returning to the group without cleaning and sanitizing the diapering surface.Plan of Correction: Same teacher is trained in handwashing and sanitizing all contacted areas
