A subsidy health and safety (SHSI) inspection was conducted on August 26, 2024. There were six children present with one caregiver staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations.
Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on
(September 11, 2024). A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-790-250-B Description: Repeat
Based on record review and discussion, the vendor did not ensure that all caregivers shall complete the Virginia Preservice Training for Child Care Staff within the first 90 days of employment.
Evidence:
The record of household member #1, employed on 06/01/2020 and identified as the substitute provider, did not contain documentation of the completion of the Virginia Preservice Training. Standard #: 8VAC20-790-250-F Description: Based on review of caregiver records, the provider did not ensure caregivers who work directly with children completed, in addition to preservice and orientation training, at least 16 hours of training to include the department?s health and safety update course annually.
