Comments:
This inspection was conducted by licensing staff using an alternate protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated and concluded on June 15, 2021. The provider was contacted by telephone to initiate the inspection. There was 1 (3 points) child present and one caregiver present. The inspector emailed the provider a list of items required to complete the inspection. The inspector reviewed 2 children's records and 2 staff records submitted by the facility to ensure documentation was complete. Standard #: 22VAC40-111-180-A
Description: Based on a review of staff records, the provider failed to obtain a current report of tuberculosis screening every two years form the date of the first screening.

Evidence:
Caregiver #1 does not have documentation of a current tuberculosis test or screening in her records.

Plan of Correction: I will schedule an appointment today
