A subsidy health and safety (SHSI) inspection was initiated on May 2, 2022 and concluded on May 3, 2022. There were 28 children present with nine staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-665-580-H Description: Based on record review, the center did not obtain the department's health and safety update course for each staff member.
Evidence:
1. The record for staff #3 did not contain the updated Health and Safety training for 2022 (training date 4/02/19).
2. The record for staff #4 (hire date 3/17/02) did not have annual health and safety update training.Plan of Correction: Not available online. Contact Inspector for more information.
