**As discussed with the main office during the file review on January 22, 2020, please ensure that staff are keeping a copy of age verification documentation in their files (i.e. copy of an ID, drivers license, etc.).
**Discussed with staff to keep children away from the mechanical wall that opens the gymnasium and ensure children stay away until it is repaired by the school's maintenance staff over the weekend.

An unannounced Monitoring Inspection was conducted today from 3:30pm to 4:20pm. There were 20 school-age children directly supervised by 3/4 staff. The physical plant, 2 staff records, 5 children?s records, 6 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed arriving to care, socializing, participating in group and individual activities, and chatting with the licensing inspector. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-560-F Description: Based on review, the facility did not ensure that when they choose to provide meals and snacks, they are indicating and listed any substituted foods.
Evidence: Reviewed the SACC's menu and found that the menu stated that they were having mini subs and Capri Suns, but instead the children will be given milk and chesse crackers.Plan of Correction: Will post any changes to the menu on the board.
