Comments:
An unannounced monitoring inspection was conducted on 4/9/24 from 8:55am to 10:20am with program staff. There were 25 children in care, supervised by five staff. The children were observed transitioning to the playground, playing outside, and doing ?zones?, which were craft projects. Five child records and five staff records were reviewed. The program has multiple staff with current certification in CPR and First Aid, and multiple staff trained in Daily Health Observation. Three medications and authorization forms were reviewed, and the program has multiple staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, allergy list, and emergency drill log were reviewed. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 4/19/24. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 8VAC20-780-40-M
Description: Based on observation and interview, the program did not maintain, in a way that was accessible to all staff who work with children, a current written list of all children?s allergies, sensitivities, and dietary restrictions in each room or area where children are present. Evidence: On 4/9/24 the program was hosting a teacher work day with two different locations. Staff were unable to locate an allergy list for the visiting school?s children during the inspection.

Plan of Correction: The visiting site posted their allergy list so it is accessible to all the sites staff.

Standard #: 8VAC20-780-510-F
Description: Based on review of medication and medication authorization, the program did not ensure that medication authorization was available to staff during the entire time it was effective. Evidence: On 4/9/24 a medication was observed on-site for a child, but program staff were unable to locate the medication authorization during the inspection.

Plan of Correction: The medication authorization form was added to the medication binder and child's medication bag.
